$\mathrm{LiAlH}_4$ is a very strong reducing agent. It will give $\mathrm{H}^{\circ}$ ion to the carbonyl carbon and reduce the carbonyl bond into $-\mathrm{OH}$ group, $\mathrm{D}_2 \mathrm{O}$ acts as a electrophile and attacks $\mathrm{O}^{-}$of hydroxyl group.
