The correct answer is: (A)-(iv); (B)-(iii); (C)-(ii); (D)-(i)
During prophase, the nucleolus and nuclear membrane disappear and spindle grows between the centrosomes. The chromosomes get arranged at the equator of the cell during metaphase by the formation of spindle poles oppositely. Centromere splits, sister chromatids are broken down and chromatids are separated during anaphase and daughter chromosomes start moving towards the opposite poles. Nuclear membrane, nucleolus and ER reappears during telophase, thus two nuclei are formed.
